SYMPLEFSI is a contemporary social care organization providing support, education, rehabilitation, and social inclusion services to vulnerable groups. Operating as a Day Care Center for People with Disabilities (ΚΔΗΦ ΑμεΑ), it was established in 2022 in Western Macedonia and maintains facilities in Kastoria, Kozani, Grevena, and Neapoli Kozanis (where a Creative Activity Center for People with Disabilities—ΚΔΑΠ ΑμεΑ—also operates).
The organization supports individuals with congenital, developmental, intellectual, motor, sensory, or mixed disabilities. Unlike clinical care structures, Day Care Centers focus on holistic, person-centered therapies that promote autonomy and social integration. SYMPLEFSI offers physiotherapy, occupational therapy, speech therapy, psycho-social support, and creative activities, tailored to the needs of each beneficiary and their family.
SYMPLEFSI’s vision is to become a model structure for the provision of high-quality services for individuals with disabilities, investing in interprofessional collaboration and modern digital technologies. Through its collaboration with smartHEALTH, SYMPLEFSI embarked on a comprehensive digital transformation effort to upgrade its care and transportation services through innovative ICT solutions.

The Challenge
SYMPLEFSI is responsible for delivering coordinated care, therapy, and transportation services to individuals with disabilities, procedures that require intensive daily organization. The challenges identified included:
- Absence of a unified digital system for appointment scheduling, beneficiary management, and communication among professionals.
- Time-consuming transportation processes, without dynamic route optimization.
- Limited capabilities for real-time monitoring of therapeutic plans and informing families.
- Necessity to securely manage sensitive personal data following a human-centered and accessible approach.
A digital platform would significantly enhance SYMPLEFSI’s operations by reducing inefficiencies, streamlining care processes, and improving service quality. Automation would release valuable staff time, allowing professionals to focus on direct beneficiary support. Moreover, optimized transportation and scheduling would reduce operational costs and ensure more effective use of resources.
The proposed technological approach—leveraging machine learning, intelligent scheduling, and cloud-based tools—could revolutionize SYMPLEFSI’s service management, positioning it as a pioneering example of research-driven innovation in social care.
The Analysis
Within the smartHEALTH program, experts from the University of Western Macedonia (UoWM) conducted a comprehensive analysis of SYMPLEFSI’s operational environment. This involved:
- Mapping existing workflows and organizational processes.
- Conducting a digital maturity assessment using the European Commission’s DMA Tool.
- Identifying functional, non-functional, and regulatory requirements.
- Defining specifications for interoperability, security, cloud management, and scalability.
- Documenting user needs across administrative teams, therapists, transport personnel, and beneficiary families.
This phase revealed a clear need for a centralized digital solution that seamlessly integrates appointment scheduling, therapy management, transportation routing, and secure data access—all aligned with GDPR requirements.
The Solutions
smartHEALTH assigned UoWM experts to design and document a proof-of-concept digital platform for coordinated care and transportation services.
The solution focused on three main dimensions:
- Non-functional requirements, including GDPR compliance, security, and information protection.
- Software architecture, supporting cloud-based management, mHealth integration, and intelligent scheduling.
- A functional prototype, enabling automated appointment management, optimized transport routing using AI algorithms, therapy monitoring, and role-based data access.
Experts also proposed operational scenarios covering:
- Scheduling and managing therapies and appointments
- Automated and optimized transportation routes
- Integration of mHealth tools for monitoring
- Cloud-native infrastructure design
- Coordination workflows among staff, therapists, and families
Consulting support was provided throughout the process, preparing SYMPLEFSI for the digital transition.

Phase I – Digital Maturity Assessment
Using the EC DMA Tool, experts evaluated SYMPLEFSI’s digital readiness and identified improvement areas. The results formed the basis of a detailed digital transformation action plan, defining strategic steps towards modernizing care operations.
Phase II – Platform Prototype Development
UoWM developed and documented a comprehensive proof-of-concept platform for managing care and transportation services. The prototype:
